How you will contribute:
The RN Case Manager is responsible for providing end-of-life care and hospice services to pients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal pient care.
This role requires compassion, strong clinical judgment, excellent communicion skills, and the ability to support pients and families through highly sensitive and meaningful experiences.
How you will make an impact:
• Assume primary responsibility for the pient/family caseload th includes assessing, planning, implementing, and evaluing care needs during on-call hours in collaborion with the triage team
• Report On-Call activities to the clinical leadership and Branch RN Case Managers the end of the call shift
• Maintain and complete approprie care coordinion notes for visit notes, and other pertinent clinical informion
• Assist, provide, or arrange for delivery of medicions and supplies needed after business hours, weekends, and/or holidays
• As scheduled, tend interdisciplinary team meetings during on-call period for full team discussion, consultion and evaluion of pient/family needs
• Under the physician’s order, admits pients eligible for hospice services in collaborion with the triage team
• Reports pient stus and need for other disciplines to clinical leadership, tending physician and hospice physician
• Develops pient care plan th specifically addresses identified pient problems; nursing problems and goals th has been identified as mutually agreed upon goals with pient(s) during on-call hours, weekends and/or holiday(s)
• Be responsible to ensure the use of the 4Ms (Wh Mters to the pient, Medicions, Mention, and Mobility) and provide Age-Friendly Care
